Forms

To apply for an online gaming license the following forms must be submitted:

1. Online Gaming Application Form
2. Business and Corporate Information Form
3. Personal History Disclosure Form

The last form form must be submitted for every qualifying person such as:

• Ultimate beneficiary owners who own 10% or more in the applicant company directly or indirectly
• Directors/Managing Directors
• Key persons such as Chief Executive and Compliance Officer

Th forms must me completed in full and signed where it is required.
